I would call your local office and self refer. I would make sure to explain your situation and see if you could have a conversation with the counselor that would most likely be assigned to you. The person that answers the phone and takes your intake does not have an idea of who would might be assigning to you, you could ask to speak with the district supervisor.

Call 1 (800) 592-6004 and tell them that you want to reopen your case. They will refer you to an RSB counselor, who will help you complete most of the paperwork over the phone. You’ll need to have a current I exam. The counselor also will come out to your house, to get more details on why you need to reopen your case. I did this about a year ago. It took about a month.
