[NFBNJ] Details on Lyft Ride Coupons for COVID-19 Vaccinations for NFBNJ members

Greetings NFBNJ members,

We have an exciting partnership with Lyft through their LyftUp Vaccine
Access program to provide access to free and donated rides to COVID-19
vaccination appointments.

Note: Lyft service levels can vary by region and time of day; please plan
your trip accordingly. Please add the code to the Rewards section of your
Lyft app right before you call your ride for your vaccination appointment.

Please do not share or post on social media. Limited quantity available.
Max 1 redemption per user. Valid for rides through 4/30/21 and cannot be
combined with other ride credits or offers. Must be used for rides to and
from vaccination appointments. Subject to Lyft's Terms of Service

<https://nfb.org/civicrm/mailing/url?u=20690&qid=5108533>, anyone who
suspects they may have or is diagnosed with COVID-19 should not use
ridesharing.

In order to use the code, individuals who are approve must:

   - download and install the Lyft mobile application (“Lyft App”) on a
   compatible mobile device
   - create and maintain an active Lyft account, including agreeing to
   Lyft’s Terms of Service.
   - successfully redeem the code in the Rider’s Lyft App
   - take a standard ride on the Lyft App

(Note: for a ride taken by a rider that exceeds the amount of the Lyft
code, Lyft will charge the rider’s personal payment method on file for the
remaining cost of the ride.)

As the New Jersey affiliate president I am only permitted to give this code
to current dues paying members of the national Federation of the Blind of
New Jersey. Members from other states will need to receive the code from
their affiliate president. The definition of a current dues paying member
as someone who has already paid their 2021 dues to either a chapter or
division of the NFBNJ.

To request this code please send an email to Linda melendez at
president at nfbnj.org with the following information:

   - Full name
   - chapter or division you belong to
   - Town and county you reside in
   - Vaccination site name, town and county
