JOB RESPONSIBILITIES:

Determines eligibility, designs and implements specialized programs of rehabilitation services for the older blind in Charleston and surrounding areas. Coordinates community resources to meet consumer needs. Provides necessary rehabilitative and medical services to include counseling, low vision, and applicable training to consumers and their families.

MINIMUM AND ADDITIONAL REQUIREMENTS:

A Bachelor's degree and professional experience in human services or social services programs.

PREFERRED QUALIFICATIONS:

Agency Requirements: A Master's degree in Social Work, Human Services, or Rehabilitation Counseling. Experience in blind rehabilitation and/or aging services is highly desired, but not required.

ADDITIONAL COMMENTS:

Considerable knowledge of the case management process. The ability to work with older adults with disabilities. Ability to communicate courteously and effectively with the public. Employee should be able to work independently. Employee will be responsible for meeting all designated productivity standards in a quality manner with minimal supervision. Travel is routinely required. Occasional overnight stays may be required. Ability to lift and move adaptive equipment in the course of providing case services.
