Good afternoon,
My name is Rebecca Weber from the National Federation of the Blind of Pennsylvania. I am emailing to ask if you wouldn’t mind completing some questions regarding your participation in the Pennsylvania Primary Election held on June 2, 2020. There are 10 questions and one question asking if there are any comments about your voting experience. Lynn Heitz, President of the National Federation of the Blind of Pennsylvania, asked if this survey could be completed by the end of June. If you wish to discuss your answers over the phone, you may call 484-648-0498. You can also send an email to nfbpvotingsurvey at gmail.com with your answers.
The questions are listed below:
1. Are you able to vote either by mail or in person without any assistance from a sighted person? Yes or No
2. Did you vote in the PA Primary Election? Yes or No3.
3. If you did not vote, please choose from the following:
a. Unable to apply for a mail-in ballot
b. Unable to get to the poling place
c. Concerned for my health and safety
4. Did you apply for a mail-in ballot? Yes or No
5. Were you able to complete the application for the mail-in or absentee ballot without any kind of assistance? Yes or No.
6. Did you physically go to your polling place? Yes or No
7. Did the polling place have an accessible voting machine? Yes or No.
8 Was the accessible voting machine hooked up and ready to go when you arrived at the polling place? Yes or No.
9. Did the poll workers seem knowledgeable about how to use the accessible voting machine? Yes or No.
10. Did you feel as though your health was in any way in jeopardy or compromised at the polling place? Yes or No.
11. Please include any additional comments you have about the voting experience:
