GRACE PAVILION COMMUNITY OUTREACH (GPCO)
and ENCINO FAMILY DENTISTRY
are now taking applications for their FREE Dental Outreach for Saturday
June 29, 2024

SATURDAY, JUNE 29, 2024 from 10AM - 2PM

Fo FREE dental cleaning or extraction.
You can apply using this link-

You are required to apply to be eligible to receive service at this event.
Applications close by Friday May 20th, 2024 before midnight.

The following documents are needed
- ID showing Applicants are at least 18 years old.
- Proof of income
- Complete the Medical History form found on the website

Note that  Applications are approved on a first come first serve basis.
