Hello all,

I am sincerely sorry for sending the form so many times. I saved the file
correctly but somehow when it was being opened it was not accessible and I
have no idea why. I am sending a Rich Text Format which should be accessible
to everyone if not, please email me and I we will come up with another

Also because of the delay in getting this out and not giving anyone much
time, I am extending the deadline to next Tuesday November 9, 2010 plus we
will have an hour of registration before the seminar begins.
