In conjunction with the Office of the Governor Ted Strickland, a group of state and private entities formulated a brief survey for persons with disabilities to complete regarding employment and support services for individuals with disabilities.  Please make every effort to offer RSC consumers an opportunity to complete the survey.  Deadlines for survey responses are June 1, 2009.   
Attached is a survey intended to be filled out by individuals with disabilities to help assess the employment services and supports for individuals with disabilities across Ohio. Your participation will aid in gaining a better understanding of the gaps in the existing employment supports and services for people with disabilities and help to enhance the system to better serve customers. 
Your participation in this survey is voluntary. You may decline to answer any question and/or withdraw from participation at any time.
