Hi Jerry,
Yes, that's definitely one way around it. Some people will want to use the
attached document, though, since it is already formatted for Word, so I
wanted to let Eric know so he can post an editable version.

I copy, cut and pasted the survey from my outlook express email to a
microsoft word file. I was able to complete the survey without any problem
from there.
