Please let me know by June 21, 2019 if you are intending to sell items at
the convention exhibit table. Here is the information I need to know:

1.	Name of chapter or division
2.	Contact name and cell number
3.	Item being sold
4.	Price per item
5.	Names of volunteers for 2 hour shifts
