Dear members,

 

We have received lift codes to assist in getting people to and from the
location to get the vaccine if you or someone that you know needs a ride,
please contact me off list.

 

Note: Lyft service levels can vary by region and time of day; please plan
your trip accordingly. Please add the code to the Rewards section of your
Lyft app right before you call your ride for your vaccination appointment.

We will not share or post on social media. Limited quantity available. Max 1
redemption per user. Valid for rides through 4/30/21 and cannot be combined
with other ride credits or offers. Must be used for rides to and from
vaccination appointments. Subject to

CDC, anyone who suspects they may have or is diagnosed with COVID-19 should
not use ridesharing.

In order to use the code, individuals who we approve must:

*     download and install the Lyft mobile application ("Lyft App") on a
compatible mobile device

*     create and maintain an active Lyft account, including agreeing to
Lyft's Terms of Service.

*     successfully redeem the code in the Rider's Lyft App

*     take a standard ride on the Lyft App

(Note: for a ride taken by a rider that exceeds the amount of the Lyft code,
Lyft will charge the rider's personal payment method on file for the
remaining cost of the ride.)

We will use discretion in assigning the code and explain that the code
should not be shared with anyone. If you are aware of someone who could
benefit from this assistance, refer them to me or Suzanne.

We want to make the process as simple as possible, while exercising care to
protect the code, and to collect information that would be helpful to us.
When we are assigning the code, we will need to collect the following
information:

*     Name of individual requesting the code:

*     Their city and state:

*     Description of how the code will be used:
