Good morning,
I have also previously had difficulties locating accessible consent form documents in a variety of settings. I have currently emailed CVS accessibility department to ask them for insight on how to locate accessible documents as I know that they do employ individuals with visual impairments, so this may be something that they have in a different location. I will share any information that I receive upon any notification from CVS.
