[Perform-talk] scholarship

Hi all,

Does anyone know how I can obtain a copy of the performing arts
scholarship form?  I emailed about it according to the instructions a
while back but did not receive a response, and I've been looking
around but have been unsuccessful in finding a copy.  As I check my
dotted Is and crossed Ts for the national scholarship competition I
was reminded of this opportunity as well.  Any assistance would be
appreciated.
