[Pibe-division] information opthalmologist include in eye report for students who are visually impaired and have additional disabilities

Hi Everyone:
The CSE wants eye reports from my students that have additional disabilities in addition to being visually impaired. What information should an ophthalmologist include on each studentsa eye report? I had a hard time accessing an eye report to see the information myself since it was in a pdf document and presented ina graphical image. Jaws for Windows could noot convert the text into speech.
