Heather, it's easy to find the application materials if you go to the
Chicago Lighthouse website and follow the links.   

Kelsey, the application is in PDF format, and not in the kind of PDF that
you can fill out.  That makes it impossible for a blind person to fill out
the form without the assistance of a sighted person.  Do you have the
application in Microsoft Word or some other format that a blind person can
fill out independently?

thank you so much for sharing this terrific information with us.  I am
currently a graduate student, and I would like to apply for this
scholarship.  Unfortunately, I was unable to gain access of the scholarship
application using the web address that you provided us with.  I received a
message stating that the page was unable to be found.  Therefore, can you
please email me an attachment of both the Chicago Lighthouse scholarship and
vision verification form?  Your time and insight is greatly appreciated.
