Several months ago BPDD received a 5 year, Wisconsin Living Well grant and
we have been working hard to get the project ready to go. There are two
positions available that have just been posted on Wisc.Jobs. We would like
your help getting the word out to apply! The first position is the *Living
Well Project Manager*, this will be a full-time job with benefits. The
second position is the *Communications Specialist* which will be a
part-time job with benefits.

*The Wisconsin Living Well project implements a 5-year systems change
statewide project to:*
1) build capacity to increase health/safety and improve quality of life.
2) develop a comprehensive monitoring effort.
The overall goal is to increase health, safety, independence and well-being
of people with intellectual/developmental disabilities (I/DD) living in the
community.
*The objectives are to:*
1) implement an intervention package in 5 pilot communities.
2) provide coaching to pilots to ensure full implementation and fidelity.
3) create policy and practice recommendations.
4) develop a statewide peer leadership network to maximize independence and
conduct health/safety monitoring.
5) determine a sustainable model of capacity building and community
monitoring to benefit individuals with I/DD living in the community.
*Outcomes:*
Anticipated outcomes include improved coordination among partners,
increased capacity of stakeholders to self-assess and monitor
health/safety, increased implementation of evidence-based practices and
community monitoring, and increased health, safety, and well-being of
people with disabilities. The project will develop two tool-kits, one on
HCBS quality improvement and the other on self-monitoring and assessment
for individuals, families and direct supports; a blueprint for an HCBS
service model based on findings; issue and policy briefs, research
articles; interactive website; and permanent peer leadership network.
