[Social-sciences-list] Accessible survey builders?

I know that in the past the survey taker side of Survey Gizmo was 
accessible, but I don't know about the creation side.
Unfortunately, this is all too common an occurrence, output side ok, 
creation side, or back-end inaccessible.
