There are a lot of ideas on the use of things like italics and bold
nowadays, and generally, most editors and publishers discourage using
either frequently.

Traditionally, italics should be used to indicate thoughts, however, if
a thought is followed by a "he or she thought," you do not use italics,
or you may use italics to emphasis a word or phrase. Punctuation
typically is not included in the italics or bolding for that matter.

The use of bold is usually to make something stand out more, like
chapter or section headings, or points on a bullet list, etc. I would
use bold less than italics if using it for reasons than bolding a title
or heading.

By references, what do you mean? Like a bibliography? If you want to
properly cite work, check out the OWL Purdue website or a writing style
guide book or Google MLA or whatever style you prefer.
