Bridget,
I tend to use these sparingly. I never use bold in the body of a work and I
restrict italics to things like the names of books or publications that show
up in the text. My understanding is that people have gone a bit overboard on
these things. I also don't care for putting words in all caps. Let the words
themselves create the emphasis.

There are a lot of ideas on the use of things like italics and bold
nowadays, and generally, most editors and publishers discourage using either
frequently.

Traditionally, italics should be used to indicate thoughts, however, if a
thought is followed by a "he or she thought," you do not use italics, or you
may use italics to emphasis a word or phrase. Punctuation typically is not
included in the italics or bolding for that matter.

The use of bold is usually to make something stand out more, like chapter or
section headings, or points on a bullet list, etc. I would use bold less
than italics if using it for reasons than bolding a title or heading.

By references, what do you mean? Like a bibliography? If you want to
properly cite work, check out the OWL Purdue website or a writing style
guide book or Google MLA or whatever style you prefer.
