[stylist] Membership Fee

HI Robert,

What forms can I get Slate and Style in?  Also, it seems to be the case 
that I signed up for a monthly payment of ten bucks, which I probably 
can't afford.  Do you know I can back out of the monthly payment and do 
the once per year thing?
