The editorial team from Colorado is David and Leonore Dvorkin.
They are DLD Books. They specialize in working with blind writers.
My 2nd book turned out to be absolutely stunning. The cover alone has a 
presence that sells the book before one even opens it, usually.
I'll be using them for my 3rd book which is now in development with my 
end-date goal of December to get it off to the  editors.

I think the key to your answer is in whom you find as an editor, and
therein is an interesting conundrum. I just published my anthology on
Smashwords.com but ended up hiring a Finishing Editor through a
writing group I belong to. This person has medical editing/publishing
experience, but took this on as a one-time project because she needed
the funds and I needed an editor fast and snappy. My other editor had
health concerns befall that prevented her from completing our
agreement. I know someone here likes a couple from Colorado who have
published blind authors, and their name escapes me. Someone else uses
Lulu.com. As stated, until convinced, the answer is in the editor. By
the way, you can get the job done for maybe 50% of what the experts
say is the going rate if you network, connect, and "do your time" in a
solid writing critique group like the Internet Writing Workshop. I
tried a hire your own freelancer and almost got screwed. I believe my
"higher power" too had a hand in editor choice in the end!
