A lot of that depends on your mail program and how it handles stuff. I could get to it easily with outlook 2010, and had a little more problems with Eudora, but it was there.

I found it by clicking the link for it, however, it was kind of difficult to get to it and I clicked open and my computer downloaded it anyway. Then, I couldn't get it to open because it was zipped and when I did the normal thing to unzip, I could not find it. Anyway, Thanks for pasting it. I will just keep that. It looks like a great assessment! Thank you for sharing and I'll look at it more closely to see if there is anything I might add or comment on.

Yes, the same thing happened on the other list I sent it to. I have pasted the text from it below.

MAB Assessment for iPad / iPhone revised July 2013

1.	Previous computer experience:

__________________________________________________________________

__________________________________________________________________

2.	Does client currently use any access technology such as JAWS or Zoomtext?

3.	Does client want to rely on vision (Zoom) or voice (VoiceOver)?

4.	Does client want to use the device outside of the home? (If so, client will need access to wifi or will need to buy a data package).

5.	Can client afford the monthly cost associated with the device?

iPhone: __ Yes __ No

iPad: __ Yes __ No
?

6.	On the iPad, can client see to navigate by touch without using Zoom?

7. Can client read text on iPad if font size is increased? (Note: this only works in Mail, Contacts, Messages, Calendar, and Notes, and if too big it can overlap).

8. Does changing to white on black (Invert Colors) make a difference?

9. When using Zoom on the iPad, can pt keep his/her place when scrolling to track lines of magnified text?

10. Can pt hear and understand the voice when using the Speak tool to listen to selected text?

11. If the client only needs speech on demand, does he/she have the manual dexterity to select text so the Speak tool can be utilized? A user has to be able to drag to select the text in Safari, since there is no ?select all? in Safari like there is in Mail. Also, a two-finger tap will select a paragraph in iBooks on the iPad, but not on the iPhone.

12. On iPhone, can client hear and understand the VoiceOver voice?

13. Does client seem to have the physical capability to use gestures?

 14. Does client seem to have any difficulty learning/remembering?

15. Does client have a family member or friend to assist with things like entering contacts if they don?t want to (or can?t) do it independently?

16. Any reasons that pt would be unable to use the iPhone or iPad? (Physical dexterity issues, memory issues, pt only has basic needs that do not require the use of complex technology, etc.).

_____________________________________________________________________

Goals:

__ Making phone calls (can be done on iPad, too, using Magic Jack)

__ Facetime or Skype (Skype may be preferable since the other person does not have to have an iDevice)

__ Surfing the internet for general information

__ Online shopping (websites and/or Amazon app)

__ Using email

__ Texting

__ Calendar (appointments)

__ Reminders

__ Recording voice memos

__ Magnifying text

__ OCR

__ Identification/labeling

__ Navigation (GPS) (actual usage in public would be done with an O&M instructor)

__ Color identification (apps are not always accurate)

__ Listen to Podcasts (include this if we are going to recommend they listen to Apple
	Vis for learning purposes)

__ Syncing to iTunes on a PC (This would only be for people who can already use a PC with Zoomtext or JAWS)

__ Taking pictures (we may not be including this)

__ Word processing/document creation for school or work (we may not be including this)

__ Accessing and listening to music (we may not be including this)

__ Does client have any computer related goals that CANNOT be accomplished on an iPhone or iPad?

If yes: ____________________________________________________________

__________________________________________________________________

__________________________________________________________________

Home setup:
1.	Does client own a PC, or will the device be used as their primary computer?

2.	Would client like to hook up to an external monitor?
__ existing VGA monitor (requires VGA adapter) __ existing HDMI monitor (requires an HDMI adapter) __ existing HDMI TV (requires an AppleTV device) __ none of the above; client needs an HDMI TV and an Apple TV device

3.	Does client already have a wireless network set up?

Password: ______________________

4.	Would client like to use an external keyboard?

5.	Does client already have an email address?

6.	Does client need the ability to print?
	Already own a wireless printer?

7.	Does client already have an Apple ID?

	If yes: ___________________________________________

Data package:

1.	If use iPad, do they want a data package to use if they are somewhere without wifi?

2. What cell phone carrier do they prefer to use?

__ Verizon __ AT&T __ Sprint __ T-Mobile

Equipment recommended

__ Tactile screen protector for iPhone from SpeedDots:

__ Standard (full keyboard)

__ Advanced (only F and J plus and buttons)

__ Protective case (such as Otterbox or Life Proof)

__ External wireless keyboard:

__Stand Alone __ Built into case (Chester Creek has one with LP keys)

__ Compass-type stand

__ Magnetic overlay keyboard for iPad

__ HDMI TV

__ Apple TV

__ Stylus

__ HDMI adapter

__ VGA adapter

__ wireless printer

Possible app recommendations:

__ VIA (recommends apps specifically of benefit for people with vision impairment)

__ Bigger and Brighter (magnify, adjust lighting and contrast, reverse brightness, take
still image and make adjustments to it)

__ Text Detective for OCR (must be used in landscape orientation)

__ Image to Text for OCR (can use in portrait orientation, image is EMAILED)

__ Tap Tap See (take a picture, it uploads to a database, and in a few seconds you get a response telling you what the item is. Will also identify money).

__ VizWiz (take a picture, record a question about it, real people respond to your question)

__ DigitEyes (audio scanner and labeler, bar code sort of like the Pen Friend that also enables you to make your own labels)

__ An app for printing from a regular printer (Printer Pro, Print Direct, etc.?whatever works with the person?s wireless printer).

__ Fleksy (predictive typing app; makes typing easier for VoiceOver users)

__ Amazon (works well with VO; simple to navigate)

__ Access World (learn about blindness-related technology)

__ NFB Newsline

__ App for NLS BARD (when it becomes available)

__ Voice Dream Reader (will also read Bookshare books for 99 cents more)

__ Read2Go (for reading Bookshare books)

__ ColorVisor or ColoredEye (whichever one we decide is better/simpler)

__ Bible Gateway (easy to use app to read the bible?can search for scriptures and bible topics)

__ Around Me (to find out what things are around you)

Note: Other navigation apps such as Navigon, Blindsquare, Ariadne, Sendero GPS, or Google Maps would be taught through an O&M program by the mobility instructor, not as part of an AT program.

Actually, attachments are allowed, and passed on this list. Except for digest mode, where they are stripped and a link is palaced in the message.
