[Trainer-talk] Report Format and Task List

Hello,

I'm writing to inquire about which format you or your agency utilizes
for your reports. Is anyone using excel, or is the standard to use a
word document with imbedded forms and/or tables? Would you be willing
to share a sample format? And is this the same for your task list and
daily progress notes? Has anyone attempted to set this up to work in
either excel or as a web page or maybe through a database?
