In my opinion, there is no "standard" at least here in Wa, any format is
fine, so you can use excel or word and create tables. I always did the
latter...

Hello,

I'm writing to inquire about which format you or your agency utilizes for
your reports. Is anyone using excel, or is the standard to use a word
document with imbedded forms and/or tables? Would you be willing to share a
sample format? And is this the same for your task list and daily progress
notes? Has anyone attempted to set this up to work in either excel or as a
web page or maybe through a database?
