Thank you.  I couldn't get make tapping and holding work in touch typing mode, but double tapping and holding did work in standard mode.  Very helpful.

According to an Apple Support article, the plus sign is dialed by tapping and holding the Zero key on the phone’s keypad.  Using VoiceOver in standard typing mode, this becomes a double-tap and hold.

With a bit of practice, I was able to successfully do this.
