I honestly cannot recall a time when the "save configuration changes on
exit" option was not the NVDA default, but, I never used NVDA at all during
the Windows XP era, and that becoming the default could very well have been
a later change.

Do what you were doing and make sure you manually save the configuration
profile after having done so.

Gemini indicates that changed default occurred at NVDA 2017.3, but that
could, of course, be in error:

NVDA officially changed its default behavior to automatically save the
configuration profile on exit starting with *version 2017.3*, released in
August 2017.

Before this update, users had to manually save their configuration (usually
via the shortcut NVDA+Control+C or through the menu) or explicitly check a
box in the General settings if they wanted changes to persist across
sessions.

The NV Access development team made this shift to align NVDA with the
behavior of most modern screen readers and standard software applications,
reducing the frustration of users accidentally losing their customized
settings when restarting their computers or closing the program.

If a user prefers the old behavior—where changes are discarded unless
manually saved—the option can still be toggled in the *General Settings*
category under *"Save configuration when exiting NVDA"*.
