Makes sense to me. If there isn't a form created by the agency in computer format for you to edit and fill out, then you can make up your own. I did that when I was doing my internship at the local Salvation Army. We were doing intake for  information and referral services primarily. The form the state required to be filled out was not accessible to me so my supervisor and I worked out a form that I could memorize and write down the specifics. As long as it was legible she didn't care if it was in 32 point magic marker. She just put it into the computer system. Since this was an internship it was a temporary solution and work for us. In a permanent one like yours I would look for permission to create an editible form you could quickly fill in, print and share with the appropriate social worker.
