[nagdu] crowded trains

That may be what they are picking up on.
I've had people who don't know me well tell me they can tell when I have a cold or sinus congestion based on how I hold my head and how I walk.  People are pretty observant, the regulars have probably noticed when you could use the seat. Since the dizziness isn't constant, they may just be overcompensating. Just politely say no if you don't want the seat on a given day.

I would have to say I doubt the others on the train have figured out when
Marsha can stand and when she can't. Maybe, though, I'm not giving them
enough credit.
