[NAGDU] American Airlines animal sanitation form for flights lasting 8 hours or more

Good morning all. American Airlines has a form that people working with service animals must complete if they will be on a flight that lasts 8 hours or more. The form requires the handler to answer the following two questions:
1.
Describe what steps have been taken to prevent your animal from relieving itself during the flight.
2.
If your animal needs to relieve itself during the flight, describe how you would handle this to prevent a health and sanitation issue.

I reviewed this form and although it is readable with JAWS, it appears that it is a PDF that is not fillable and that someone filling it out would have to print it, fill it in by hand, and sign it. This of course could be problematic for guide dog users who need to complete the form but do not have sighted assistance.

Is anyone aware of an accessible version of this form and if not, have you been able to get assistance from American Airlines if needed in order to complete the form? Thanks for any experiences you can share.
