[NAGDU] Florida Tax Exemption

It took a bit of digging, but the form comes from the Department of 
Revenue (attached). It cites the Florida Department of Blind Services 
Bureau of Blind Services, which, of course, does not exist. They 
accepted the silly little self written ID that DBS gives out, but it 
took a bit of prodding. They finally got a bureau chief involved, who 
was quite cordial.

The form that the revenue folks issue is DR152.

I'll write something up for our Florida folks, if you'd like.
