Sometimes it’s just lack of expectations.  If you’ve never had to do 
it or been expected to do it, you may not have learned techniques for 
how it’s done.  I admit that I had trouble with the concept the first 
day at the Colorado Center for the Blind under sleepshade.  Someone 
offered me a couple of tips and I picked up the skill pretty quickly 
thereafter.

But then I already knew how to do it, essentially, I just had to 
learn a different method.
