Gary, I had little problem filling one out. Now, I get to put one in. I found a great site out of Russia, but the department seems to like primates.

Good afternoon David. I have used Survey Monkey to complete surveys, but never have I tried to offer a survey using it. My recollection is that it isn't the friendliest program to work with accessible technology but if one knows how to manipulate combo boxes, edit boxes, and easily slip in and out of forms mode, it isn't much of a problem.
