Mike,
I hate to say this, but from the way that drive is acting, it doesn't 
look like a write-protect problem. It honestly looks like a flash chip 
gone bad or in the process of doing so. Write-protection wouldn't cause 
the 0 bytes thing or the "this drive needs to be formatted" thing. It 
would cause the write-protect error upon attempting to format, but not 
the rest of it. You might run chkdsk on the drive, just to see what it 
says, but if this problem is what I think it is you'll almost certainly 
get and error when you try to do that.
