[NFBCS] Creating a Form

Hello:

I have a document that I would like to convert to a web form so that it 
can be completed and signed using DocuSign. The document contains fake 
lines where the person would fill in the information. Any suggestions on 
how to tackle this project? DocuSign allows you to create a web form. It 
talks about uploading a .json file. Any and all input on tackling this 
project would be appreciated.
