Hi Leslie,
If you have a Word file, you could upload this file to Perplexity and prompt it to convert to an HTML file for you. I've never used the AI service for a web form, so I would be especially careful to look over the code and certainly test the form before disseminating, but for plain copy, it's great. Every now and then I'll get a random ` symbol in the output, but just prompt again, tell it to remove extraneous characters, and the results are great.

Subject: [NFBCS] Creating a Form
Hello:
I have a document that I would like to convert to a web form so that it can be completed and signed using DocuSign. The document contains fake lines where the person would fill in the information. Any suggestions on how to tackle this project? DocuSign allows you to create a web form. It talks about uploading a .json file. Any and all input on tackling this project would be appreciated.
