PDF files have to be laid out correctly in order to be accessible.  The OCR
Software that comes with Adobe Acrobat ( for instance) is not very helpful
if you have two columns of text in an image of a document, you get to read
the first line of text for each column and then onwards down the page. If
you can, try to avoid PDF files for accessibility, Adobe does not want to
make any new innovations on this subject, they are not open to new ideas.
 Also the PDF standards board is run by a guy who sells the software that
the US government uses to make all of its documents so there is an interest
to keep things the way they are, don't change.
