I reached out to Amazon on Twitter and got the following reply from them:

@AmazonHelp: @SalisburyJustin Hi! For Amazon Smile to work, you would need to order off of the Amazon Smile site itself.

So, yes, it appears that you need to order off of the website in order for the Amazon smile program to take effect.

I am not technically well versed, but I try. So I finally went to
Smile.amazon.com<http://Smile.amazon.com> and put NFB of CT for my charity for Amazon to donate
.05percent of my purchases. On my iPad I have the Amazon app, but I beleive
I,have to go to smile.amazon.com<http://smile.amazon.com> to order now in order to get the donation.
Is this correct?
