In this case it may be best to stick with the tried-and-true low-tech solutions: a Perkins Brailler (my preference) and/or a slate and stylus. If you want to go the electric route, you can use Braille screen input on your iPhone as Edward suggests, although I've found it to be unreliable in many cases. So, I might suggest using a VoiceNote or VoiceSense, or simply using speech on a Braille notetaker.
