I have filled out the form and only had someone work with me because I wasn't
certain that the area for under 60 was "required". I used Google Chrome with
JAWS on my Windows 10 machine. Lynn told me that she used her iPhone so it may
be something in settings on your computer or something else.

If you have difficulty completing the application due to access issues, contact
Microsoft Accessibility, push 1 for tech support and 2 for windows.  They are
generally good at helping with difficult applications and websites.

Perhaps it's just my less than great tech skills, but when I visited this
website and tried to click on the very important questions about using the
service, I couldn't bring up the answers. Probably some of you will have better
luck.
