Hello,

Every 3 years, we are required to conduct a Comprehensive Statewide Needs
Assessment. It’s a chance for our agency to review the effectiveness of our
VR services and resources.

We collect data in a few ways, including through surveys.

Below is a link to a survey for the individuals we serve to complete.

I hope you will be willing to take this survey by Saturday, April 18.

The feedback we collect is essential in identifying areas of strength and
opportunities for enhancement.
