Every 3 years we are required to conduct a Comprehensive Statewide Needs
Assessment. It’s a chance for our agency to review the effectiveness of our
VR services and resources.

We collect data in a few ways, including through surveys.

Below is a link to a survey for the individuals we serve to complete.

It is my hope you would be willing to take this survey by Saturday April 18.

The feedback we collect is essential in identifying areas of strength, and
opportunities for enhancement.
