Statewide Needs Assessment 

Survey Deadline Extended!

The Ohio Statewide Independent Living Council is currently working to gather
input from the disability community and those that are involved with the
disability community. We are currently gathering feedback until September
2nd, 2022. The link to the survey is listed below. 

We appreciate your time and input. Your feedback will help us in the writing
of our State Plan for Independent Living, education efforts, and increasing
awareness to the needs of the disability community in Ohio. 

If you need assistance in completing this survey, would like a mailed copy,
or would like to complete the survey in any alternate format please contact
