[NJTechDiv] report Lyft ride never occured

Hi all,

Technology really doesn't like me tonight.  I couldn't find the Help menu
in the Lyft app to save my life!  I even uninstalled the app and
reinstalled it.  Luckily, Lyft also has a Contact Us section on the
computer with many of the options Rick described to me on the phone.  I
found the service animal denial number.  (Although I didn't have an issue
with a service animal, since I'm blind and there's no option to describe my
exact situation, Rick told me to call the service animal denial number.)
