I haven't used Google forms for that, but have for other purposes. It works fine, save that if you are doing multiple choice questions, the check boxes don't check with a screen reader. Use the drop down instead. Exporting to sheets is easy, and data displays like an Excel spreadsheet. You may need help getting things to print if you need them to.
