[Ohio-talk] fire safety survey

Greetings, 
I was wondering if anyone tried the fire safety survey sent the other day.  I went to the site, provided by the link, and there was just 1 question.  Normally I've completed surveys where there is a multiple choice question with checking off a check box.  I didn't see any way of making my selection.  I'm curious what the others encountered.
