[AERNet] need assistance regarding obtaining medical information to include in annual progress report

Hi, last evening I wrote about what components a vision teaccher includes in an annual progress report to the CSe. I understand I must include present level
of performance, annual goals and objectives related service being provided duration and frequency of related service and recommendation. I finally received
some medical information about some of my students which is the specific visual condition but not any acuity level. Anyone know if this sufficient information
to present to the CSE when writing my annual progress reports. Do I have to explain each visual condition in lay man terms? Also, do I need to include
acuity levels which the individuals that provided the information did not have written in the eye report? Thanks for any ideas. My students are multiple
disabilities in addition to being visually impaired or blind. I work as an independent related service provider providing vision education services at
a non-public school. I am not really guided how to write anual reports by my school. This is why I am asking for assistance from this list. Thanks for
any assistance one can provide to me. Just to note that my students are nonverbal and nonambulatory besides being visually impaired or blind and I service
one student who has autism is nonverbal and ambulatory. Thanks for any suggestions. My apologies for a lenthy email.
