I think a lot of people use Skype.  In conjunction with that, For JAWS
users, Tandem is great.  There's also the Serotalk solution which costs
money if you aren't training an SA user but which is probably quite good.

Is anyone currently conducting any long distance, and/or online AT classes?
I've been asked to organize a way of providing AT training services
throughout the state of Idaho, without traveling all the time. I'm
wondering what people are using, and some of the advantages and
disadvantages you have encountered in setting up and using online
based training systems.
