[Trainer-Talk] CREATING ACCESSIBLE, FILLABLE FORM IN WORD

Hello All:

I'm stuck on this one and in dire need of some helpful suggestions!  Using
Word2007.  

A new requirement in my line of business has to do with sending out several
forms to customers; these forms are to be filled out and returned to my
company.  I have SAMPLES OF MOST OF THESE FORMS, but they ARE NOT
accessible.  

I am willing to RECREATE THESE FORMS and apply all accessibility hooks using
Word.  Now, I don't know where to start or how to go about accomplishing
this task.

I'd be interested in hearing from anyone who has successfully created an
accessible, fillable form of any kind.  For the forms I need, there will be
need for controls like edit boxes, combo boxes, radio buttons, checkboxes
and signature line.  I'm open to taking this off list especially if what I
am trying to do is far more complicated and the need to avoid list clutter
is of absolute importance.
