Greetings to all!

We have provided the pre registration form (full and short versions) pasted
and attached for your reading pleasure.

If you print, remember the pre registration form is larger than normal and
the form is 8 1/2 by 14 inches, legal size.

When you complete, I just need the part of the form with information
pertaining to your name, email, phone, what you are registering for, type of
meal for the banquet, etc.

Please make sure you notify me of your choice of media for the agenda,
print, email or Braille.

The pre registration form will be posted on Newsline and our state website
and has been distributed to all on our announcement list.

Please share with others.  Our state convention is open to all.
