[OAGDU] Reporting service animal denials to Uber and Lyft.

I have been doing this when I am denied a ride but I need to do much better
reporting when everything is going well.  I have also been talking to Uber
and NFB about the lack of improvement and the treatment of drivers when I
report. I doubt Uber ever terminates any driver for denying a ride to a
person with a service animal.
